When you get an incident report
Where the report lives, what signing it means, and what to do if you have questions.
How you'll hear about it
Bumps, bites and falls happen in every room. When one is worth writing up, your center tells you directly — usually at pickup, or by phone if it needs you sooner. That conversation is the point; the app is the written record that follows it.
When your center sends the report, you'll also get a notification on your phone and an email. Neither says what happened — they tell you a report has been filed and to open the app, because an injury description doesn't belong on a lock screen or in an inbox. The report itself is behind your own sign-in.
Finding the report
On the day, the incident appears in your child's day on the Today tab, with a short description.
Any time after that, it's on your child's profile:
- Open the profile Go to More and tap your child's profile, which is labelled with their name. You can also tap Profile on your child's card on the My Child tab.
- Look under Medical The Medical section opens on its own. Scroll to Incident reports.
What the entry shows
Each report is listed by your child's name, Incident Report and the date. Underneath, the dates it was signed appear as each step happens: when the teacher signed it, when a parent acknowledged it, and when the director signed it off.
The tag tells you where it has got to, and whether anything is waiting on you:
- Needs your signature — the report has been sent to you and hasn't been acknowledged yet.
- Signed — with your center — a guardian has acknowledged it; your director still has to sign it off.
- Closed — the director has signed it off. Nothing further is needed.
Under the tag you'll see what happened, in the center's words, with the date, time and place. If you'd like to go through it in more detail, ask your center — it's a reasonable request, and easier face to face than by message.
Acknowledging it
- Tap Acknowledge incident It's under the report.
- Sign Sign with your finger in the signature box.
- Tap Sign You'll see Incident acknowledged. and the report now shows the date you signed.
Acknowledging confirms you received the report. It isn't a statement that you agree with every word of it — if you don't, see below.
One acknowledgment covers the report. If another guardian has already signed, the button won't be there for you.
Signing at the center instead
Some centers still hand you a device to sign at pickup. That counts: it's recorded as signed in person, with the staff member who was there as the witness, and the app won't ask you to sign again.
After you've signed
Your director reviews the report, signs it off, and it's marked Closed. It stays on your child's record afterwards — incident reports are licensing records, so your center keeps them.
If you have questions or disagree
Talk to your director, and do it the same week while everyone still remembers the day. Ask what happened, who saw it, and whether anything is changing in the room as a result.
If you believe something in the written record is wrong, you can also ask for a formal correction — see Download, correct or delete your child's data.
The Medications card on your child's profile shows what your center has recorded — the medication, dose, schedule and the dates it covers. If one needs your written authorization before staff can give it, the card says so, and you can sign for it under Health → Medications. See Authorizing a medication.
